Each level contains a number of dream crystals, which, when collected, give points to make the game's high score. An extra life can be gained by collecting 100 dream crystals. Extra lives can also be found in the form of hidden "Lollipop" items.

Enemies can be defeated by jumping on them or shooting them after obtaining the relevant power-ups. The enemies include owls, rolling eyeballs, flesh-eating fish and deadly insects. The "fire wheel" transforms Giana into a punk with the ability to crush rocks by jumping and hitting them from below. The "lightning bolt" awards Giana "dream bubbles", a single projectile shot. "Double lightning" gives her the ability to shoot recoiling projectiles. "Strawberries" give her the ability to shoot homing projectiles. There is one defensive item in the game, the "water drop", which protects Giana against fire. A number of special items can also be triggered that affect the entire screen, such as the "clock", which freezes all enemies on-screen, and the "magic bombs", which kill all enemies. These items are found in the item blocks scattered around the stages.
Game Description: The game pits up to four players (or AI opponents) against each other in local and online competition, speed-running for the finish in an effort to push each other off the screen with the help of boosters and power-ups. In Giana Sisters: Twisted Dreams, This side-scroller 2.5d platformer Giana can swim, jump, kill enemies by jumping on their heads or 'twist' the dream, and collect gems to unlock next the levels. When Giana twists, the entire dream world is transformed, changing everything in the surroundings right away and it can be performed at any moment in any level. In the Nightmare version, Cute Giana is peaceful, mocking the nightmare which once frightened her when she was a child, and with peaceful music in the background. Cute Giana can twirl, spin in the air, and fall very slowly. Unlike in previous games, Giana no longer need a Fire Wheel to transform into her "punk" persona. In the Dream version, Punk Giana is upset and the soundtrack is turned into metal music arrangements. Punk Giana can bash, reach high passages, destroy weak obstacles and kill enemies. Some obstacles are only present in one world, or moving platforms change their direction when Giana twists. At the end of each world there is a boss to beat: Giana has to avoid its attacks and find a way to hit it three times.
Building on the concept of Giana's own transformation, the team began to draw a concept for Giana to have the ability to change the world around her and herself. Several prototypes were made to test the trans-morphing of the environment, enemies and Giana's own abilities. The concept was considered to be a success, and Black Forest Games began to focus the game to make use of a transformation system which is controlled by the player itself. The engine was built up entirely in-house in order to provide a player-controlled switch affecting the entire environment, as well as the in-game audio which has two distinctly different sounds depending on which dream world is being played.
It's a very simple gameplay mechanic that ends up being a lot of fun. It flows smoothly and seamlessly, and the game is clearly designed to be something that both casual players and speedrunners can enjoy. Finishing a stage on its own is enjoyable, but doing so in a smooth unbroken mix of hops, bounces and shifts makes you feel incredibly cool, even when it's not that challenging. The game offers time attack challenges, which emphasize how smoothly the game allows you to chain together abilities to finish stages quickly.
